The Exchange Visitor J-1 non-immigrant visa is for applicants who wish to take part in work and study-based exchange and visitor programs in the U.S.
J-1 exchange visitors come to the United States as part of the US graduate medical education programs or for training at US medical schools, attend clinical exchange programs, to teach, study, receive training, or demonstrate special skills.
These programs are sponsored by an educational or other non-profit institution, which must be accredited through the Exchange Visitor Program designated by the U.S. State Department.

No. You would need to pay a new visa fee and apply for a new visa.
